INDIVIDUAL DENTAL & VISION INSURANCE
We offer a variety of dental plans for 2016. Some require you to go to only the contracted dentists (PPO), and some allow you freedom to go to any dentist (Indemnity). I explain the 'Pediatric Dental' requirement at the bottom of the page.
MODA HEALTH IS OFFERING 3 PLANS UNDER THE NAME DELTA DENTAL.
If you have had 12 months of prior coverage Moda will waive the waiting period for Basic and Major coverage.
1. DELTA DENTAL EPO PLAN: $31 per month
EPO means you can only see dentists in the The Delta Dental EPO Network . The plan pays 100% Class 1, Preventative , 80% for Class 2, and 50% major, with a $1000 maximum annual benefit. This plan satisfies the Pediatric Dental requirement. NO out of network benefits
2. DELTA DENTAL PPO PLAN: $28 per Month
If you dentist is NOT in network this plan allows you to see any dentist. Coverage pays 75% Preventative, 60% Basic, and 50% Major with a $1000 annual maximum. To view the Moda brochure click on the link below:
3. DELTA DENTAL PREMIER PLAN: $39 per Month
The Premier Plan covers almost 95% of all dentists in Oregon. There is an annual $50.00 deductible. Coverage pays 100% Preventative, 70% Basic, and 50% Major with a $1000 annual maximum. To view the Moda brochure click on the link below:

SPIRIT DENTAL & VISION PLANS
The Spirit Dental plans are underwritten by Security Life. 2 plans allow you to only use providers from the MaxCare network. The other 2 plans allow you to use any dentist. Don't dismiss the network plans as your dentist may participate and you can enjoy added savings from the negotiated rates. Indemnity means you can use any dentist.
